My Delta Dryden pivoting toilet paper holder #75150 did not come with a mounting template.  Can I print one?

We have a delta dryden aged pewter toilet paper holder that is 16 years old. It came with a plastic piece on the so that the metal “arm” didn't hit on metal. Ours broke. Is juat that piece replaceable?

Can this holder be installed with a left or right pivot? We can only seem to install it as a right to left opening.The instructions are not clear. When I try to maneuver it by spinning the arm the lever appears to be upside down with the opening facing up

What kind of head is on the screw that attaches the product to the bracket?
