How far do you have to turn the valve to get fully open? IE: 1/4 turn, 1/2 turn, full turn.

The handles would depend on the trim kit used to complete the faucet. Handles typically have 1/4 turn stops.

We had this installed a year ago but has recently lost flow. Can the individual valve be removed to check for blockage?

As a tub filler, it is not designed to restrict the flow to a maximum flow rate. It will depend on the water pressure and other plumbing factors. At 60 PSI the flow rate with a tub filler trim will be approximately 8 GPM.
